Universidad Nacional de Córdoba, 2007 Summary: Luego de la terapia endodóntica el elemento dentario necesita ser protegido para prevenir el riesgo de fractura, especialmente en la región cervical, donde se produce la mayor concentración de tensiones. EI pronóstico del diente endodonciado y restaurado con endopostes va a estar influenciado por el tipo de restauración utilizada y por la cantidad de estructura dentaria remanente. EI propósito de este estudio fue evaluar, mediante el metodo de análisis por elementos finitos, el comportamiento estructural de primeros premolares superiores tratados endodónticamente y restaurados con sistemas de endopostes y coronas coladas. Se realizaron siete modelos 3D de primeros premolares superiores en un soporte óseo considerando tres niveles de remanente coronal: 1) sin remanente; 2) con 2 mm; y 3) con 4 mm. Se reconstruyeron con dos sistemas de endopostes: a) preformado de fibra de carbono con matriz de resina epoxi; b) colado de cromo-níquel y restaurados con coronas coladas de cromo níque. Un modelo fue reconstruido con resina sin restauracón a perno (control). Se aplicaron dos cargas estaticas de 100 N y 600 N, en 45º sobre la vertiente interna de la cúspide palatina. Se evaluó la concentración, localización y distribución de las tensiones (compresión, tracción, Von Mises). Los resultados mostraron que la zona cervical recibió mayores tensiones. En los modelos restaurados con endopostes, dicha zona se correspondió con la de maxima tensión observada en el perno. Los postes de fibra de carbono absorbieron menores tensiones comparados con los colados. Asimismo, se redujeron las tensiones en la dentina con el aumento del módulo de elasticidad del perno. En la raiz palatina se observaron esfuerzos de compresión y en la vestibular, de tracción. Ademas, a mayor remanente dentario disminuyeron las tensiones en dentina.Summary: Following endodontic therapy, teeth need to be protected to prevent the risk fracturing, particularly in the cervical region, which is subject to the most stress. The endodontically treated, dowel-restored tooth prognosis will be influenced by the type restauration performed and by the amount of remaining tooth structure. The purpose of this study was to assess, using the finite element analysis method the structural behavior of endodontically treated, dowel and crown restored first premolars upper premolarsSeven 3-D models of first upper premolars were created in a simulated bone basis considering three levels of coronal residual: 1) no residual; 2) with 2mm; and 3) with 4mm. They were reconstructed with two dowel systems: a) a prefabricated carbon fiber resin post; b) a NiCr cast and restored with NiCr cast crowns. One of the models was reconstructed with resin with no post (control). Two static loads of 100 N and 600 N were applied at an angle of 450 over the internal side of the palatine cuspid. Stress concentrations, positions and distributions (compressive, tensile, and Von Mises stresses were assessed. Results showed that cervical region received the most stresses. In the dowel restored models, such a region corresponded to the maximum observed stress on the post. The carbon fiber posts received less stress compared to the casts ones. Likewise, dentin stresses were reduced with the increase of the module of elasticity of post Compressive stresses were observed in the palatine root, and tensile strength in the vestibular one. Moreover, the more dental remain the less dentin stress.
