Summary: El tratamiento en dientes que no completaron el desarrollo radicular y su pulpa se ha necrosado, fue y sigue siendo un problema difícil de resolver. Con resultados inciertos, el tratamiento de apicoformación está siendo sustituido por procedimientos regenerativos, y si bien abundan estudios sobre el tema, poco se valora el terreno endodóntico. Con el fin de conocer aspectos de la microestructura dentinaria que delimita la cavidad pulpar en dientes cuyas pulpas se necrosaron antes que completaran su formación, se efectuaron 3 (tres) estudios: ESTUDIO 1. Análisis microestructural de la pared dentinaria de la cavidad pulpar en dientes en vía de formación. Se conformaron grupos según el diente presentaba: sólo la mitad de su raíz formada (Grupo 1); falta de formación del sector apical (Grupo 2); Raíz formada y conducto inmaduro (Grupo 3) y Dientes formados y maduros (Grupo 4). Sobre cortes longitudinales y mediante MEB, en la superficie dentinaria pulpar dividida en tres sectores (coronario, medio y apical), se analizaron y compararon aspectos generales de su estructura y en particular, el número y diámetro de sus túbulos. ESTUDIO 2. Análisis del contenido mineral de la dentina. Se efectuó mediante el microanálisis por espectroscopía de energía dispersiva (EDS) sobre dientes con desarrollo radicular incompleto (Grupo A) y sobre dientes formados y maduros (Grupo B). ESTUDIO 3. Análisis de la microdureza dentinaria. Al igual que en el Estudio 3, las muestras se distribuyeron en Grupos A y B. Sobre secciones longitudinales de los dientes se determinó la microdureza Knoop en tres sectores de la cavidad pulpar (coronario, medio y apical). RESULTADOS. En términos generales, tanto el análisis comparativo entre valores promedios correspondientes al número de los túbulos como al de sus dimensiones, efectuado entre los tres sectores de los dientes, mostró que entre ellos las diferencias eran estadísticamente significativas (p<0,001). En cambio, el contraste entre grupos mostró que las diferencias eran particularmente significativas (p<0,001), sólo entre grupos extremos. Esto indica que en el diente joven el grado de madurez dentinaria guarda estrecha relación con el grado de formación radicular, y que a menor desarrollo radicular, mayor el número, la amplitud e inmadurez de los túbulos. Evidencia, además, que la formación de la raíz en un diente joven (Grupo 3), no significa que posea “madurez endodóntica”, por lo que concuerda más con los grupos en vías de formación, que con el de dientes formados y maduros. En el análisis mineral la relación Ca/P fue de 1,67 en el Grupo A y de 2,08 en el B, mientras que la del Ca/F fue de 76,05 y de 122,51 respectivamente, con diferencias significativas (p<0,05) en ambos casos. Respecto a la microdureza knoop, las diferencias fueron significativamente mayores (p<0,05) en el Grupo B (55,5 HK) que en el Grupo A (42,7 HK), mientras que entre sectores las diferencias sólo fueron significativas (p<0,05) en el Grupo A. CONCLUSIONES: En términos generales, la evidente falta de maduración dentinaria, que se traduce tanto en la estructura tubular como en sus propiedades físicas, hace que la cavidad pulpar en dientes en vías de formación sea un ámbito complejo, poco conocido y escasamente valorado en el momento de diseñar los protocolos para su tratamiento.Summary: The management of teeth with incomplete root formation and pulp necrosis has always been a difficult problem to solve. Although with uncertain results, regenerative procedures are substituting apical formation, but even when there is a large number of studies on this subject, the endodontic aspect is little considered. Three studies were conducted with the purpose of analyzing features of dentin microstructure bounding the pulp cavity in teeth whose pulps became necrotic before completing their development. STUDY 1.- Microstructural analysis of the dentin wall of the pulp cavity in developing teeth. For this analysis, teeth were divided into four groups: teeth presenting only half of the root developed (Group 1); lack of development of the apical region (Group 2), a developed root and an immature canal (Group 3); and developed mature teeth (Group 4). General characteristics of the dentin pulp structure, especially the number and diameter of tubules, were analyzed and compared by means of MEB and by longitudinal sections made on the dentin pulp surface which was divided in three portions (coronal, middle and apical). STUDY 2.-.Analysis of the mineral content of the dentin. This study was done by means of microanalysis using energy dispersive spectroscopy (EDS) in teeth with incomplete root development (Group A) and in developed mature teeth (Group B). STUDY 3.- Analysis of dentin microhardness. As in Study 3, the samples were distributed in Groups A and B. Longitudinal sections were cut through the teeth to determine Knoop microhardness in the three portions of the pulp cavity (coronal, middle and apical). RESULTS. The comparative analysis between average values corresponding to the number of tubules as well as to their dimensions showed that the differences in the three portions of the teeth were statistically significant (p<0.001). Conversely, the contrast between the groups showed that the differences were particularly significant (p<0.001) only in extreme groups. This finding demonstrates that the degree of dentin maturation in the young tooth is closely related to the degree of root formation; it also reveals that the less the root is developed, the greater the number, width and incomplete maturation of the tubules. In addition, this result shows that the formation of the root in the young tooth (Group 3) does not mean it has reached “endodontic maturation”; therefore, it is more associated with the developing groups than with the group of formed, mature teeth. The analysis of minerals revealed that Ca/P-ratio was 1.67 in Group A and 2.08 in Group B, whereas Ca/F ratio was 76.05 and 122.51 respectively, with significant differences (p<0.05) in both cases. Regarding the knoop microhardness, the differences were significantly greater (p<0.05) in Group B (55.5 HK) than in Group A (42.7 HK). With respect to the sections, differences were significant (p<0.05) only in Group A. CONCLUSIONS: In general terms, the evident lack of maturation of the dentin observed in the structure of the tubule, as well as in its physical properties, makes the cavity of the pulp of the teeth in development a complex area, little known and little valued at the time of designing treatment protocols.
