Universidad Nacional de Córdoba, 2014 Abstract: Introducción: Estudios previos han demostrado que los bisfosfonatos, como Alendronato de sodio (AL), son potentes inhibidores de la resorción ósea y aumentan la densidad mineral del hueso. El fluoruro administrado como Monofluorfosfato de Sodio (MFP) estimula la formación e incrementa el volumen del hueso, efecto específicamente debido a la estimulación de la actividad osteoblástica. Objetivo: Evaluar el efecto del tratamiento combinado de AL vía subcutánea y de MFP vía oral sobre la regeneración tisular de cavidades óseas neoformadas en tibias de ratas. Materiales y métodos: Las fórmulas farmacéuticas se prepararon con una dosificación de 0,5 mg/kg de peso para AL y de 5 mM para MFP. El ensayo de citotoxicidad se realizó por medio del Método de captación de rojo neutro. Este colorante penetra en las membranas celulares y se fija a la matriz lisosomal. Es posible así distinguir entre células viables, dañadas o muertas de acuerdo a la acumulación del rojo neutro dentro del lisosoma. Para este ensayo se utilizó la línea celular VERO cuyas células fueron cultivadas en un Medio Esencial Mínimo (MEM) suplementado con 10% de suero fetal bovino a una densidad celular de 70% a 80% de confluencia. La viabilidad celular se estimó a través de la extracción del colorante presente en vacuolas celulares. Para analizar la toxicidad de MFP se realizaron ensayos “in vivo”. Se midió peso corporal, glucosa sanguínea y relación ceniza/matriz. Se emplearon sesenta y cuatro ratas macho de la línea Wistar de peso 160 ± 20 g, y se dividieron en 4 grupos de 16 ratas cada uno. El grupo C recibió semanalmente 0,3 ml/100g de peso corporal de solución salina vía subcutánea cercana a la intervención quirúrgica y actuó como grupo control. El grupo A recibió semanalmente 0,5 mg de AL/Kg de peso corporal por vía subcutánea profunda en el miembro posterior izquierdo. El grupo M fue tratado con MFP en el agua de bebida durante el tiempo que duró el experimento y en las áreas de la cirugía los animales recibieron una inyección subcutánea de solución salina como el grupo control. El grupo AM recibió tratamiento combinado con AL subcutáneo y MFP por vía oral en el agua de bebida. Estos animales tuvieron acceso a agua corriente de red “adlibitum”. El tiempo total de la experiencia fue de 90 días. En la cirugía se realizó una incisión longitudinal en ambas tibias y a través del decolado se llegó a exponer el hueso, realizando un defecto circular en la parte plana de cada tibia hasta llegar al hueso medular. Dicho defecto no fue rellenado con ningún material. Se tomaron radiografías de cada tibia a 0, 15, 30, 60 y 90 días y fueron analizadas con el Software Image Pro Plus versión 4.1 de Media Cibernetics. También se evaluó fosfatasa alcalina (FA) en sangre mediante métodos colorimétricos en los mismos tiempos. Los animales fueron sacrificados a los 15, 30, 60 y 90 días. Se recolectaron ambas tibias y además los fémures para ser utilizados en ensayos biomecánicos. Se realizaron estudios histopatológicos previa descalcificación de las tibias con EDTA y su inclusión en parafina. Los cortes fueron teñidos con HE y observados con microscopía óptica (MO). Los estudios estadísticos se realizaron a través del análisis de la variancia a dos y tres criterios declasificación (tratamientos, tiempo, tibia problema/tibia contralateral). El nivel de significación fue p<0.01. Resultados: Se observó que a concentraciones menores a 10 μg/ml de AL la viabilidad celular fue cercana al 100 %. A medida que aumentaron las concentraciones por encima de los 10 μg/ml la viabilidad celular comenzó a verseafectada, aunque nunca por debajo del 70%. No se hallaron diferencias significativas entre el grupo tratado con MFP y el grupo control en las variables de toxicidad “in vivo” analizadas. Los estudios radiográficos demostraron que AL desde el día 0 comenzó a aumentar la densidad óptica (OD), siendo el valor más alto a los 60 días, declinando hacia el día 90, donde los valores se estabilizaron. MFP se comportó de manera similar a AL, aunque la OD promedio fue menor. El tratamiento combinado incrementó la OD pero con valores menores al de las drogas por separado. No hubo diferencias significativas entre tibia problema y contralateral. Los estudios de fosfatasa alcalina (FA) revelaronun aumento gradual de este marcador en los grupos problemas con respecto al control. El análisis histológico demostró que AL y MFP presentaron mayor actividad osteogénica observándose trabéculas más gruesas y anastomosadas, con mayor relevancia en el tiempo 60 y estabilizándose al tiempo 90. La histomorfometría reveló un aumento del porcentaje de hueso trabecular a través del tiempo para AL y MFP siendo el más evidente el tiempo 60. El tratamiento combinado no fue estadísticamente diferente al grupo control. Los ensayos biomecánicos demostraron para AL y MFP un aumento en la rigidez a los 60 días. El tratamiento combinado no mostró diferencias significativas para dicho parámetro. En cuanto a la fuerza de fractura y la energía absorbida, no hubo diferencias entre los grupos. Conclusiones: AL vía subcutánea a bajas dosis no es citotóxico y MFP por vía oral a dosis de 5 mM no afecta la viabilidad celular. El aumento de FA se corresponde con un aumento de la actividad osteoblástica. AL y MFP aumentaron la OD significativamente como así también el número y grosor de las trabéculas analizadas al MO. La rigidez fue aumentada por AL y MFP. El tratamiento combinado de AL y MFP no demostró sinergismo en los tiempos de la experiencia probablemente debido a un efecto antagónico. And that fluoride, such as Monofluorophosphate (MFP), estimules bone formation and increases bone mass. This effect is specifically caused by a proliferation in the osteoblastic activity. Objective: In this research the effect of the combined treatment of AL, subcutaneously, and of MFP, supplied orally, over the tissue regeneration of neo-formed bone cavities is studied. Materials and methods: The pharmaceutical formulations with a dosage of 0.50 mg/kg of the weight for AL and 5 mM for MFP were prepared. The cytotoxical trial was made by means of the Neutral Red Method. The cellular line VERO was used, its cells were cultivated in a Minimun Essential Medio (MEM) with 10% of fetal bovine serum to a cell density from 70% to 80% of confluence.The cell viability was estimated by measuring enzymes present in cell vacuoles in living cells. To analyse MFP cytotoxicity “in vivo”, tests were developed measuring corporal weight, glucose blood and the relationship ashes/matrix. Sixty-four Wistar male rats 160 ± 20 g of weight were used, and the samples were divided into 4 groups of 16 rats each one. A group acted as a group control (C). The animals of this group received a subcutaneously dosage of saline solution of 0.3 ml/100g of the corporal weight given weekly, injected near the surgery intervention zone. A second group (A) received a deep subcutaneously AL dosage of 0.5 mg of the corporal weight in the left leg. A third group (M) received a treatment with MFP in their drinkable water, during all the research time, and was injected subcutaneously with saline solution near the surgery intervention zone, same as (C). A fourth group (AM) received a combined treatment of AL injected subcutaneously plus a MFP dosage in their drinkable water. All these animals had access to running water “ad libitum”. At surgery a longitudinal incision was made in both tibias and the bones, through taken off were exposed, a circular defect in the flat part of each tibia was done, until the medullar bone was reached. This defect was not filled. Radiographs of each tibia were taken on the 1 ,15 ,30 , 60 and 90 days of the research and were analyzed with the Image Pro Plus Software version 4.1 of the Media Cybernetics. Alkaline Phosphatase (ALP) was also tested in blood, by coloring measured methods, during the time periods mentioned above. The animals were sacrificed on the 15 , 30 , 60 and 90 days of the research. The tibia and the femur were kept to be used in biomechanical trials. Histopathological studies were performed with a preview tibia decalcification with EDTA and embedded in paraffin. The sections were stained with HE and observed with optical microscopy. The comparison of the data was conducted by analysis of variance with two and three criteria of classification (treatment, time and tibia problem/tibia contralateral). The level of significance was p<0.01. Results: It was observed for AL that to smaller amount concentration than 10 μg/ml, the cell viability was near to 100%. When there was a higher concentration, above 10 μg/ml the cell viability showed a difference, although it was never below 70%. There were no significant differences for the cytotoxicity in the variables “in vivo” for the MFP. AL radiographic studies showed that from day 0, it began increasing the bone mineral density (BMD) average, finding the peak at day 60, declining to day 90, where a plateau level was reached. MFP showed a similar behavior than AL, although BMD average was lower. The combined treatment increased the average BMD but with lower values that the drugs incorporated alone. There was no significant difference between tibia problem and the contra lateral. ALP analysis showed a gradual rise in the groups A, M and AM compared to group C. Histology data revealed an improvement in the osteogenic activity, showing thicker and anastomosed trabecule for AL and MFP, with a higher relevance in day 60 and finding a plateau in day 90. Histomorphometry revealed a rise in the trabecular bone percentage through time for AL and MFP getting the most evident peak on day 60. The combined treatment did not show significant differences to the control. The biomechanical trials found its highest rigid point on day 60. The combined treatment was not significant. There were no differences among the groups who were exposed to a biomechanical load breaking bone point or to the absorbed energy measurement. Conclusions: AL placed under skin at a low dosage is not cytotoxic, and MFP via oral under a dosage of 5mM, does not affect the cell viability. An increase in ALP corresponds to an improvement in the osteoblastic activity. AL and MFP rose not only the BMD, but also the number and the thickness of the analyzed trabecules with MO. Bone rigidness was incremented by AL and MFP. This combined treatment did not show synergism during the research due to an antagonic effect.
