Universidad Nacional de Córdoba, 2001 N2 - Se estudiaron las propiedades biológicas y osteoconformadoras de filamentos de titanio agrupados en forma irregular constituyendo lo que se denominó: esponja de titanio, posteriormente se evaluó clínica y radiográficamente la osteointegración de implantes con carga protética insertados en hueso que tiene incorporado en su organización estructural, esponja de titanio. Para ello se emplearon en la faz experimental 20 conejos machos, de aproximadamente un año de edad, en los cuales se tallaron cavidades óseas de 3x3x7 mm en ambas tibias. Se aplicó esponja de titanio cubierta con membrana de colágeno en las tibias derechas, dejando las izquierdas como testigos. Reintervención a los 30 dias para la inserción de implantes en hueso con esponja de titanio y testigo. Para la investigación clínica 15 pacientes, de ambos sexos, de 18 a 65 años agrupados de acuerdo al número de paredes alveolares perdidas. Se aplicó esponja de titanio post-extracción en todos los casos. Reintervención a los 60 días para la inserción de implantes en alvéolos osteoconformados con esponja de titanio. La histología convencional por transparencia con inclusión en resina epóxica, demostró la presencia de hueso en todas las superficies de la esponja de titanio cuando se aplicó adecuadamente aireada y en un ambiente óseo. Estos detalles histológicos, corroboran la biocompatibilidad de la esponja de titanio la cual no fue expulsada en ningún caso. La sección de filamentos de titanio observada por MEB muestra interfases titanio hueso homologable a la oseointegración de implantes de titanio. La dureza del hueso (microdureza Vickers) y los ensayos biomecánicos de tracción sobre los implantes mostraron diferencias no significativas con los controles. La inserción de implantes permitió una adecuada técnica quirúrgica y fijación primaría del implante no siendo obstáculo la dureza del hueso y la presencia de filamentos de titanio incluidos en él. La evaluación clínica radiográfica en humanos demostró la capacidad de la esponja de titanio para conformar ad hoc el tejido óseo alveolar perdido logrando reforzar su organización estructura. La oseointegración y el comportamiento clínico implanto-protético fue controlado como exitoso en seguimiento de hasta 4 años; Biological and bone modeling properties of titanium filaments have been studied. These filaments are gathered irregulary, which constitutes the titanium sponge. Afterwards, there was a clinical and x-Ray evaluation of the osseointegration of implants inserted in bone with prothetic load that has titanium sponge incorporated in it structural organization. For this purpose, 1) Experimental case study of twenty male rabbits of about one year old were chosen. Bone cavities of about 3x3x7mm were curved in both tibiae of each animal. Later, titanium sponge covered with collagen membrane in the right tibiae was applied; the left tibiae were left as control. For clinical research: fifteen patients: nine male and six female from 18 to 65 years old. The patients were gathered according to the number of lost alveolar walls. Then, titanium sponge post-extraction was applied in all cases. Sixty days later, the pacients were re-operated to insert the implants in the bone alveolar conformation with titanium sponge. Conventional histology has demonstrated the presence of bone in all surfaces of titanium sponge when applied properly aerated and in osseous environment. All these histological details confirm the biocompatibility of the titanium sponge which has not been expelled in any case. The section of titanium filaments observed by SEM showed interfaces of titanium bone that are similar to the osseointegration of titanium implants. The bone strength (micro hardness Vickers and the biomechanical traction tests made on the implants did not show significant differences with the controls. The insertion of the implants allowed an adequated surgical technique and primary fixation of the implant, but the strenth of the bone and the presence of titanium filaments were not an obstacle to the operation. The X-Ray evaluation in human beings has demonstrated the capacity of the titaniurn sponge to conform the alveolar bone tissue lost. The osseointegration and the clinical behavior of the prothetic implant were successful in all the tests made during a period of four years ER -
