Anexos N2 - En las primeras semanas de vida intrauterina (VIU), se producen malformaciones orofaciales congénitas como la fisura labio alveolo palatina (FLAP), que cursan con anomalías dentarias relacionadas a etapas odontogénicas en dentición temporaria y permanente. En la 6° semana de VIU se produce la proliferación del epitelio oral de ambos maxilares formando las láminas dentarias y a partir de éstas los botones dentarios. Dicho proceso comprende una serie de cambios químicos, morfológicos y funcionales, conocido como Ciclo Vital de Desarrollo y Crecimiento dentario, con sus diferentes etapas de crecimiento, calcificación y erupción. Numerosas contingencias ocurridas en estas semanas de VIU pueden causar anomalías dentarias tales como agenesia, supernumerarios, microdoncia, macrodoncia, fusión, geminación, hipoplasia, hipomineralización y erupción ectópica, entre otras. El presente trabajo se propone identificar, cuantificar y analizar clínica y radiográficamente anomalías dentarias originadas en el ciclo vital que afectan la dentición de pacientes con fisuras (PF) maxilofaciales del Servicio de Atención Interdisciplinaria (SAI) a pacientes con FLAP y compararlos con una población de niños sin fisuras (PNF) que asistieron a la Cátedra de Integral Niños y Adolescentes (INA) todos de la Facultad de Odontología de la Universidad Nacional Córdoba, excluyéndose a los niños con enfermedades sistémicas graves y niños con fisuras asociadas a síndromes. Se analizaron datos clínicos y radiográficos obtenidos mediante I. Historia Clínica: filiación, antecedentes biológicos y patológicos sistémicos y particulares del sistema estomatognático. II. Examen Clínico: inspección sistematizada de la cavidad bucal, evaluando las características morfológicas e histológicas de los elementos dentarios. III. Examen Radiográfico: Ortopantomografías y periapicales. Muestra n=100 PNF grupo control y n=100 PF grupo problema. En grupo PF la distribución según sexo fue 47% varones 53% mujeres y en PNF 46 y 54% respectivamente. En relación a las anomalías dentarias en grupo control, se observó en la serie permanente: mal posición y/o rotación dentaria 62%, asimetrías eruptivas 45%, asimetrías dentarias 26%, ectopìas 27%, agenesias 12%, hipomineralización 48% y supernumerarios 7%. En la serie primaria: hipoplasias 19% y fusión 1%. Las anomalías dentarias del grupo problema PF en serie permanente fueron mal posición y/o rotación dentaria 100%, asimetrías eruptivas 80%, asimetrías dentarias 89%, ectopìas 82%, agenesias 51%, hipomineralización 69%, supernumerarios 18%. En la serie temporaria: hipoplasias 47%, supernumerarios 11%, agenesias 4%, fusión 1% y ectopìas 3%. De los datos generales para grupo problema se observó que las fisuras eran: 43% Bilateral, 37% Unilateral izquierda completa, 13% Unilateral derecha completa y 5% Palatina, de ellas 78% completas y 22% labio-alveolar. En pacientes no fisurados se registró un mayor porcentaje de anomalías en dentición permanente y en maxilar superior con alta prevalencia de malposición y/o rotación dentaria, hipomineralización y asimetrías eruptivas. Se determinaron diferencias significativas entre sexos en hipomineralización. En el grupo pacientes fisurados se observó alta incidencia de anomalías del desarrollo del ciclo vital del diente en ambas denticiones. La distribución según tipo de fisura resultó un mayor porcentaje coincidente con fisuras unilaterales. Siendo los elementos dentarios más afectados incisivo central y lateral izquierdos. Se observaron en todas las variables mayor porcentaje de alteraciones para el grupo pacientes no fisurados con respecto a las variables del grupo pacientes no fisurados; the first weeks of Intrauterine Life (IUL), congenital orofacial malformations such as, the cleft palate lip alveolus, occur with dental anomalies related to odontogenic stages in temporary and permanent dentition. In the sixth week of Intrauterine Life, proliferation of the oral epithelium of both jaws takes place forming the dental laminae and from these the tooth buds. Said transformation comprises a series of chemical, morphological and functional changes, known as the Vital Cycle of Development and Dental Growth. Understanding this as the dynamic process of growth, development and maturation of the dental element in its different stages of Growth, Calcification and Eruptive with its corresponding phases: Initiation, Proliferation, Histodifferentiation, Morphodifferentiation, Apposition, Mineralization, Eruption and Root Development. Numerous contingencies occurred in these weeks of IUL can cause dental anomalies such as: agenesis, supernumeraries, microdontia, macrodontia, fusion, gemination, hypoplasia, hypomineralization, ectopic eruption among others. It is proposed to identify, quantify and analyze clinically and radiographically dental anomalies originated in the life cycle that affect the dentition of patients with maxillofacial fissures of the Patient Care Service to patients with CLAP and also, compare them with a population of children without fissures that attended the Chair of Integral Children and Adolescents all from the UNC School of Dentistry. Excluding children with serious systemic diseases and children with fissures associated with syndromes. Clinical and radiographic data that were obtained, was analyzed by: I. Clinical history: filiation, biological and pathological systemic antecedents, and particular of the stomatognathic system. II. Clinical Exam: systematized inspection of the oral cavity, evaluating morphological and histological characteristics of the dental elements. III. Radiographic examination: Orthopantomographies and periapicals. Shows n=100 patients without fissures control group and n=100 patients with fissures problem group. The distribution according to sex in patients with fissures was 47% males, 53% females and in patients without fissures 46% males and 54% females. It was observed in permanent series in relation to dental anomalies in control group: bad position and/or dental rotation 62%, eruptive asymmetries 45%, dental asymmetries 26%, ectopias 27%, agenesias 12%, hypomineralization 48%, supernumeraries 7%. In the primary series: hypoplasias 19%, fusion 1%. The dental anomalies of the problem group in permanent series were bad position and/or dental rotation 100%, eruptive asymmetries 80%, dental asymmetries 89%, ectopies 82%, agenesis 51%, hypomineralization 69%, supernumerary 18%. In temporary series: hypoplasias 47%, supernumeraries 11%, agenesias 4%, fusion 1% and ectopies 3%. It was observed from general data of the problem group that the fissures were: 43% Bilateral, 37% left Unilateral complete, 13% right Unilateral complete and 5% Palatine. From them 78% complete and 22% Alveolar-lip. A higher percentage of anomalies were registered in non-fissured patients in permanent dentition and in the upper maxilla with high prevalence of malposition and/or dental rotation, hypomineralization and eruptive asymmetries. Significant differences between sexes in hypomineralization were determined. High incidence of developmental abnormalities of the tooth life cycle was observed in both dentitions in the group of fissured patients. The distribution by type of fissure resulted in a higher percentage coincident with unilateral fissures. The most affected tooth elements were left incisor and left lateral.
