Universidad Nacional de Córdoba, 2004 Summary: Con el propósito de analizar in vitro la capacidad de sellado de tres materiales empleados para obliterar perforaciones artificiales del piso de la cámara pulpar en molares inferiores, se realizaron las siguientes experiencias: A) Estudio del efecto antibacteriano del MTA (Agregado de trióxido mineral) y del CP (Cemento Portland), en el que se evaluó la acción antimicrobiana de los mismos frente al Proteus mirabilis y el Streptococcus salivarius, bacterias empleadas en este trabajo; como así también, la posible contaminación que pudieran tener dichos productos. B) Estudio de permeabilidad a la microfiltración bacteriana, con el fin de investigar la capacidad de sellado inmediata y a distancia que poseen el MTA, CP y la amalgama de plata libre de zinc. C) Por último se repitieron similares experiencias a los efectos de analizar el comportamiento de una base intermedia de hidróxido de calcio debajo de la obturación. Se utilizó un método bacteriológico que permitió comprobar la migración de las bacterias citadas en sentido corono apical. Los re- sultados obtenidos en la valoración del efecto antibacteriano permitieron comprobar que el MTA tuvo una marcada acción antimicrobiana, mientras que el CP tuvo una reducida actividad sobre el Streptococcus salivarius y ninguna sobre el Proteus mirabilis. En cuanto a la capacidad selladora de las perforaciones furcales, en el estudio inmediato el MTA fue claramente superior al CP (p<0,02) y a la amalgama (p<0,05), no así en el estudio mediato donde los tres materiales investigados mostraron ser poco efectivos y además, que entre ellos las diferencias carecían de significación (p>0,05). La utilización de una base intermedia de hidróxido de calcio pareciera no influir sobre los resultados, por lo que su empleo no es estrictamente necesario. Frente a la pobre capacidad de sellado a la miecrofiltración bacteriana que demostraron los tres materiales investigados, creemos que es prudente remarcar la importancia que tiene efectuar un correcto sellado provisorio así como una adecuada reconstrucción coronada definitiva, lo que sin duda mejora el pro- nóstico en dientes que han sufrido la perforación accidental del piso de la cámara pulpar durante la realización de un tratamiento endodóntico.Summary: The purpose of this study was to analyze the in vitro sealing ability of three materails used for filling artificial perforations in the pulp chamber floor of mandibular molars. The experiment consisted of three parts: A) Study of the antibacterial effect of MTA (mineral trioxide aggregate) and of CP (Portland cement), and evaluation of their antimicrobial action against the bacteriae used in this study, the Proteus mirabilis and the Streptococcus salivarius. The possible contamination that these products might present was also considered. B) Study of the perrneability to bacterial rnicroleakage in order to investigate both the immediate and long-term sealing ability of MTA, CP, and zinc-free amalgam. C) Similar procedures were repeated in order to analyze the behaviour of a calcium hydroxide intermediate base under the obturation. The bacteriological method used showed the passage of bacteriae in a crown-down direction. The results of the assessment of the antibacterial action demonstrated that MTA had a marked antimicrobial action, whereas CP had very little effect over the Streptococcus salivarius and none on the Proteus mirabilis. With regard to the sealing ability of furcation perforations, MTA revealed a significantly better imrnediate ability than CP (p<0,02), or than amalgam (p<0,05); however, when the long-term ability was studied the three rnaterials proved to be poorly effective. In addition, there was no significant difference among them (p>0,05). The use of a calcium hydroxide intermediate base is not strictly necessary since it seemed not to have an effect on our results. In light of the poor sealing ability to bacterial microleakage of the materials studied, we believe it is advisable to highlight the importance of proper temporary sealing as well as a suitable permanent coronary reconstruction, which undoubtedly will result in a better prognosis for the teeth undergoing accidental perforation of the pulp chamber floor during root canal treatment.
